
Prevalence of Abortion and Related Factors in Ilam: A Retrospective Cohort Study

Abstract
Title: Prevalence of abortion and related factors in xxx: A retrospective cohort study Introduction: Abortion is one of the most common adverse pregnancy outcomes. Different studies have stated different causes for it. The aim of this study was to determine the prevalence of abortion in XXX and its associated risk factors. Methods and Materials: In this retrospective cohort study, 7,490 pregnancy cases registered in XXX city between 2020 and 2022 were examined for miscarriage. Data were collected in three sections: demographic information, medical and reproductive history, and information related to recent abortion using a researcher-made questionnaire. Statistical analysis was performed using the chi-square test, Fisher's exact test, and t-test. The significance level was set at 0.05. Results: Of the 534 abortions, 338 (63.3) were spontaneous and 196 (36.7) were induced-legal abortions. The prevalence of abortion was 1.7%. The mean maternal age (33/35±5/51, VS, 34/80±6/32, CI: -2/47- -0/419, P=0/006), the mean paternal age (37/26±5/92, VS, 38/78±7/03, CI: -2/69- -0/34, P=0/0012) and the mean gestational age based on LMP (12/94±7/4, VS, 15/22±4/39, CI: -3/10- -1/44, P=0/001) and based on the first ultrasound (12/12±4/70, VS, 14/30±4/63, CI: -3/03- -1/36, P=0/001) were significantly lower in spontaneous abortions than in induced-legal abortions. 51.5% of spontaneous abortions occurred at a gestational age of less than 12 weeks and 70.4% of induced-legal abortions occurred at an age of more than 12 weeks. 62.9% of abortions at less than 12 weeks were due to lack of FHR visibility and 40.1% of abortions at more than 12 weeks were due to fetal abnormalities (P=0.001). Conclusion and Discussion: The results show that some factors of miscarriage are unknown and others are known and may be preventable. These findings can help increase awareness among couples and inform the development of policies to reduce the incidence of miscarriage in the country.
